American Psychiatric Association to Integrate Biology into Next DSM Edition

The American Psychiatric Association announced that the next edition of the Diagnostic and Statistical Manual of Mental Disorders, known as the DSM, will incorporate biology to help understand mental disorders for the first time. This move aims to modernize the manual, which serves as a primary reference for clinicians and researchers worldwide. The current version, DSM 5, released in 2013, has faced criticism from professionals who argue that a lack of objective biological measures may reduce diagnostic validity and lead to the medicalization of human suffering.

Experts involved in the strategic committee note that the integration of neurobiology will follow a nuanced approach because scientific understanding of the biological basis of mental disorders is evolving slowly. While biomarkers, which are objectively measurable characteristics used in other fields of medicine to diagnose conditions like diabetes or cardiovascular risk, are widely used, they are currently absent for most psychiatric conditions. The only exception is the use of certain proteins to diagnose Alzheimer's disease.

Several factors contribute to this gap in psychiatric medicine. Biological changes in mental illness are often subtle and difficult for current technology to detect. Furthermore, severe mental illnesses are viewed as brain disorders with significant social and psychological components. Factors such as family history and trauma make it difficult to isolate biological influences from psychosocial ones. Additionally, psychiatric disorders are often conceptualized as overlapping behavioral syndromes rather than distinct diseases, meaning patients frequently meet the criteria for multiple disorders simultaneously.
